If using last names only, do not add first names. If using a combination add both. If no numbers, do not add them
Describe your design and appearance requests, such as colors, stripes, bands, logo location, etc. If no input colors will be based on your logo, and appearances will be standard as in example photos

Description

To purchase, select the number that will fulfill your team quantity. Complete the form with the NAMES and NUMBERS of the players on the team, and specify if you want FIRST or LAST name on all of them. If you do not select the default is FIRST on Helmet name tag and Waterbottle name tag, LAST on stick name tag. Also submit your logo, via the upload button ensuring your file is high resolution (preferred EPS, PDF, AI, SVG or PNG). To make custom appearance requests please enter that in the “other instructions” box and we will work to accommodate you!
